Educational Qualification: Candidates having Diploma/Degree in Pharmacy from recognized institutes and must be registered under Assam Pharmacy Council.

Scale of Pay: Pay Band-2, Rs. 14,000/- to Rs.49,000/- + GP Rs.7400

Radiographer

Total No. of post: 49
Educational Qualification: Candidates having Radiographer Technician Certificate Course or above from any Govt, recognized Institute of Govt, of Assam.

Scale of pay: Pay Band-2, Rs. 14,000/- to Rs.49,000/-+ GP Rs.6200.

Laboratory Technician

Total No. of post: 41
Educational Qualification: Candidates having Laboratory Technician Certificate Course or above from any Govt, recognized Institute of Govt, of Assam

Scale of pay: Pay Band-2, Rs. 14,000/- to Rs.49,000/-+ GP Rs.6200.

Account Assistant / Junior Assistant

No. of post: 44
Educational Qualification: Candidates having Graduate in any discipline from recognized university with certificate of computer application.

Scale of pay: Pay Band-2, Rs. 14,000/- to Rs.49,000/-+ GP Rs.7400

Dresser

Total No. of post: 48

Educational Qualification: Candidates having HSLC or equivalent passed from a recognized Board with experience.

Scale of pay: Pay Band-2, Rs. 14,000/- to Rs.49,000/-+ GP Rs.5000

Category reservation: Reservation of posts for SC / ST (P) / ST (H) / OBC (MOBC) / Women candidates and Physically Handicapped candidates will be as per Government Rules.

Age: Candidate should be in between 18 years and 43 years as on 01.01.2017. However the upper age limit is relaxable by 5 years for SC, ST (P), STH candidates only.

Application fee: The application fee of Rs.250/-(Rupees Two hundred fifty)only for General Category candidates and Rs.150/-(Rupees One hundred fifty) only for OBC/MOBC.SC,ST(P) & ST(H) Category candidates will have to be paid through Treasury Challan under the Head of Account “02 10-m&ph- 01-Urban Health Services-800-Other Receipt” and the scanned copy of the challan to be uploaded while submitted the online application form. BPL candidates are exempted from paying application fees and they are required to upload their BPL certificate from the competent authority for such exemption of fee along with the application.

Candidates interested are required to fill up the online Application Forms available at the official website of Directorate of Health Services, Assam(www.dhs.assam.gov.in)

The following documents in original will have to be produced at the time of document verification and photocopies of all certificates etc. are to be submitted.
(a) 02 (two) Copies of recent passport size colour photograph duly self attested.
(b) HSLC Admit Card or Birth Certificate for age proof.
(c) All marks sheets and pass certificate.
(d) Caste certificate (where applicable)
(e) Permanent Residential Certificate
(f) Employment Exchange Registration Certificate. Certificate of training experience / Extra Curricular etc.
(g) Pharmacy Registration Certificate authenticated by Assam Pharmacy Council.
