Directorate of Health Services, Assam, Guwahati -36 invited applications from interested candidates, who are citizens of India and having Registration Number in Employment Exchange in the State of Assam in the prescribed Standard Form of Application as given in Part-IX of the Assam Gazette, for filling up the various vacancies for 329 nos of Gr-IV posts in 47 Nos. of newly created Model Hospitals of the District level in the State.

Minimum Educational Qualification: The Candidate should be Class 8 (VIII) passed.

Scale of Pay: Pay Band-1, Rs.12,000/- to Rs.37,500/- + GP Rs.3900/-

Reservation : Reservation of posts for SC/ ST(P)/ ST(H)/ OBC(MOBC)/ Women candidates and Physically Handicapped candidates are as per Government Rules.

Age limit: Age of Candidate should in between 18 years and 43 years as on 01.01.2017. However the upper age limit is relaxable by 5 years for SC, ST (P), STH candidates only.

Application fee:

For General category candidates: Rs. 250/-(Rupees Two Hundred Fifty) Only.

For SC/ST/OBC reserved category candidates: Rs. 150/- (Rupees One Hundred Fifty) Only.
Candidates having BPL certificate- No need to pay application fees.

Fees should be deposited only through Treasury Challan in the Head of Account “ 0210-M&PH-01-Urban Health Services-800-Other Receipt ”. Candidate can download the treasury challan from the Important link section.

Detailed list of eligible candidates for each post along-with the details of selection procedures shall be notified in due course of time and shall be displayed in the website (www.dhs.assam.gov.in).

How to Apply:

The applicants should submit their completely filled in application in the prescribed Standard Form of Application as given in Part-IX of the Assam Gazette along with photocopies of all testimonials (self-attested) regarding qualification, age, caste, employment exchange registration card, Caste certificate (in case of reserved category candidates), Disability Certificate (in case of Person with Disabilities) issued by competent authority, Original Treasury Challan, copy of BPL certificate in case of candidates of BPL families and two recent passport size photograph of the candidate with his /her signature at the top. The self addressed envelope of 10’x4.5′ affixing postal stamp of Rs.5/- must be enclosed with the application.
